You can eat almost nothing and still overeat
Eighteen women were given the same meals at three different energy densities. They ate the same weight of food every time, took in 1,376, 1,519 and 1,800 calories, and reported no difference in how full they felt.
"I hardly ate anything today" is usually true. It is just not an answer to the question people think it answers.
The amount of food somebody eats and the number of calories they eat are two different measurements, and they come apart much further than most people expect. There is a study that separates them cleanly, and it is worth knowing about before you decide you have a discipline problem.
The same plate, 424 calories apart
Bell and colleagues gave eighteen normal-weight women meals across three two-day sessions. At lunch, dinner and an evening snack they had free access to a main entree that varied in energy density, the calories per gram, while being matched for palatability. Compulsory low-energy side dishes came with every meal, and breakfast was standard.[1]
The result is the whole article in one sentence. Participants ate a similar weight of food in all three conditions. Because the food weighed the same but carried different amounts of energy, they took in:

And then the part that matters most: there were no differences in hunger or fullness before meals, after meals, or across the two days.[1] The 1,376-calorie day did not feel like less food, because it was not less food. It was the same food by weight, carrying 424 fewer calories.
Same weight of food. 424 calories apart. No difference in reported fullness.
The authors drew a conclusion worth repeating, because it cuts against the usual story: energy density affected intake independent of macronutrient content, which suggests that overconsumption of high-fat foods may be down to their energy density rather than to their fat.[1] Fat is not doing something sinister. Fat has nine calories a gram and everything else has four, so fat makes food denser, and denser food carries more calories into the same volume.
People eat volume, and volume is not the same as weight
The follow-up is stranger and more useful. Rolls and colleagues gave twenty-eight lean men milkshakes that were identical in ingredients, identical in weight and identical in calories, and varied only in volume, from 300 to 600 mL, by whipping different amounts of air into them.[2]
Thirty minutes later, the men who drank the 600 mL shake ate 12% less at lunch than the men who drank the 300 mL one, and reported more fullness and less hunger.[2] The only difference between the two was air.
Whatever mechanism decides you have had enough is reading something closer to bulk than to calories. It can be moved by literally nothing.
Fat plus carbohydrate is the combination, and it is not a coincidence
Almost nothing in nature is high in fat and high in carbohydrate at the same time. Meat and nuts are fat. Fruit and grain are carbohydrate. The foods that combine them in quantity are, with few exceptions, ones somebody designed.
DiFeliceantonio and colleagues put that to a test. Using an auction task during brain imaging, they had participants bid real money on foods, and found people were willing to pay more for foods combining fat and carbohydrate than for equally familiar, equally liked and equally caloric foods containing fat alone or carbohydrate alone. The extra value showed up in reward circuitry, including the dorsal striatum.[3]
Calorie for calorie, the combination was worth more to people than either component. The authors call the effect supra-additive: the two together are valued more than the sum of their parts.[3]
The same paper carries a second finding that gets quoted far less and explains far more. People were better at estimating the energy content of fat-only foods than of fat-plus-carbohydrate foods.[3] So the combination is not only the most rewarding, it is also the one your judgement is worst at reading. That is the exact overlap where "I hardly ate anything" is honestly believed and factually wrong.
These foods now have a formal definition rather than a vibe. Fazzino and colleagues built a quantitative one, based on combinations of fat, sugar, sodium and carbohydrate above threshold levels, and applied it to the US food database to measure how much of the food system meets it.[4] "Hyper-palatable" stopped being an adjective and became something you can count.
Density is a large part of it, and not all of it
Here is where an honest article has to complicate its own thesis.
Hall and colleagues ran an inpatient randomized trial: twenty adults, admitted to a clinical center, given ultra-processed or unprocessed diets for two weeks each in random order, eating as much or as little as they wanted. The meals were matched for presented calories, macronutrients, sugar, sodium, fiber and energy density.[5]
People still ate 508 more calories a day on the ultra-processed diet. They gained 0.9 kg on it and lost 0.9 kg on the unprocessed one, and weight change tracked calorie intake closely. The extra calories came from carbohydrate and fat; protein intake did not differ.[5]
Read that carefully. Energy density was matched, and the overeating happened anyway. So density is a real and large lever, demonstrated cleanly in Bell's study, but it is not the whole mechanism. Food form, texture, how fast it can be eaten and how much chewing it demands are all still in play, and this trial cannot tell you which of them did the work.

What this actually means for you
Your fullness signal is not broken and it is not lying. It is measuring the wrong variable for the question you are asking it. It reads bulk, and it reads it well enough to be moved by whipped air. It does not read calories, and no amount of paying attention will make it start.
"I hardly ate today" and "I ate too many calories" are both true at once, routinely. They are not in conflict. One is about volume and one is about energy, and the foods that separate them furthest are the ones combining fat and carbohydrate, which are also the ones people judge worst.
This is an argument for measuring, not for restraint. If perception tracked calories, nobody would need a food log. The reason to log is that the gap between how much you ate and how much energy you took in is invisible from the inside, and it is largest exactly where the food is most designed.

This is why the food log reports calories rather than portions, and why the day's remaining calories sit on the Today screen instead of a serving count. It is also why the app never tells anybody they are eating too much: the honest version is that the amount of food and the amount of energy are different measurements, and only one of them is visible without checking.
